Balloon dilatation versus surgical valvotomy for congenital aortic stenosis: a propensity score matched study

Background:
Balloon valvuloplasty and surgical aortic valvotomy have been the treatment mainstays for congenital aortic stenosis in children. Choice of intervention often differs depending upon centre bias with limited relevant, comparative literature.
Objectives:This study aims to provide an unbiased, contemporary matched comparison of these balloon and surgical approaches.
Methods:Retrospective analysis of patients with congenital aortic valve stenosis who underwent balloon valvuloplasty (Queensland Children’s Hospital, Brisbane) or surgical valvotomy (Royal Children’s Hospital, Melbourne) between 2005 and 2016. Patients were excluded if pre-intervention assessment indicated ineligibility to either group. Propensity score matching was performed based on age, weight, and valve morphology.
Results:Sixty-five balloon patients and seventy-seven surgical patients were included. Overall, the groups were well matched with 18 neonates/25 infants in the balloon group and 17 neonates/28 infants in the surgical group. Median age at balloon was 92 days (range 2 days – 18.8 years) compared to 167 days (range 0 days – 18.1 years) for surgery (rank-sum p = 0.08). Mean follow-up was 5.3 years. There was one late balloon death and two early surgical deaths due to left ventricular failure. There was no significant difference in freedom from reintervention at latest follow-up (69% in the balloon group and 70% in the surgical group, p = 1.0).
Conclusions:Contemporary analysis of balloon aortic valvuloplasty and surgical aortic valvotomy shows no difference in overall reintervention rates in the medium term. Balloon valvuloplasty performs well across all age groups, achieving delay or avoidance of surgical intervention.

Summary
This is not a standard book introduction. Usually co-editors produce an integrated explanation of the contents and the key lessons contained in the book. We have chosen to take another approach. We have written separately to introduce the book – not because we don't get along, and not because we have difficulty writing with one another. We have enjoyed many exchanges and have offered one another much editorial assistance in producing these chapters. We have written separately to illustrate the two kinds of stories and principles that you will experience as you read the chapters. One focuses on the practical lessons of leadership – how to be a better leader and how great leaders lead with values. The other focuses on an explanation of why the outcomes occurred, what might explain the success being described, and what key concepts must be kept in mind. That is, the book tells engaging and interesting stories, but it also adds substantively to our understanding of leadership, organizational success, and how they are related through values. We begin with Ed.

This book is a symphony of wonderful stories about leaders and leadership. A particular brand of leadership – Values-Based Leadership – based upon certain universal, fundamental values, such as honesty, respect, trust, and the dignity and worth of every individual as a human being.
A values-based leader serves others in a positive way. A positive way includes the manner in which he or she leads; the way he or she treats his or her followers; and the ultimate morality of the objectives or results.

Summary
This chapter highlights leadership lessons from positively deviant organizational performance – that is, the achievement of extraordinary success well beyond the expectations of almost any outside observer. It recounts the story of an organization that reached a level of performance that was considered impossible, so that adjectives such as spectacular, extraordinary, remarkable, and astonishing are apt descriptors. This account, based on Cameron and Lavine (2006), describes how a single organization experienced a devastating loss – the loss of mission and subsequent languishing performance – and then, despite its problematic circumstances, achieved astounding success. The chapter highlights the key role leadership values played in this extraordinary level of performance, and it explains the values that leaders in other organizations can apply in enabling their own spectacular success.
Rocky Flats
On March 23, 1951, the Atomic Energy Commission publicly announced that the nation would build a top-secret nuclear weapons plant in a rocky but flat ranching area 16 miles northwest of downtown Denver, located at the base of the beautiful Flatirons on the eastern slope of the Rocky Mountains. The site began operation in 1953 and functioned until 1989 when it was abruptly closed after a raid by the FBI. Rocky Flats was owned by the US Department of Energy (DOE) and managed by a series of weapons contractors during its years of active operation: Dow Chemical (1952 to 1975), Rockwell International (1975 to 1990), EG&G (1990 to 1995).

Background. We carried out a large randomized trial of a brief form of cognitive therapy, manual-assisted cognitive behaviour therapy (MACT) versus treatment as usual (TAU) for deliberate self-harm.
Method. Patients presenting with recurrent deliberate self-harm in five centres were randomized to either MACT or (TAU) and followed up over 1 year. MACT patients received a booklet based on cognitive behaviour therapy (CBT) principles and were offered up to five plus two booster sessions of CBT from a therapist in the first 3 months of the study. Ratings of parasuicide risk, anxiety, depression, social functioning and global function, positive and negative thinking, and quality of life were measured at baseline and after 6 and 12 months.
Results. Four hundred and eighty patients were randomized. Sixty per cent of the MACT group had both the booklet and CBT sessions. There were seven suicides, five in the TAU group. The main outcome measure, the proportion of those repeating deliberate self-harm in the 12 months of the study, showed no significant difference between those treated with MACT (39%) and treatment as usual (46%) (OR 0·78, 95% CI 0·53 to 1·14, P=0·20).
Conclusion. Brief cognitive behaviour therapy is of limited efficacy in reducing self-harm repetition, but the findings taken in conjunctin with the economic evaluation (Byford et al. 2003) indicate superiority of MACT over TAU in terms of cost and effectiveness combined.
